慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)是一种具有气流阻塞特征的慢性支气管炎和(或)肺气肿,可进一步发展为肺心病和呼吸衰竭的常见慢性疾病,致残率和病死率很高[1],全球40岁以上发病率已高达9%~10%,我国 40 岁以上患病率高达 13.7%,60岁以上患病率达27%[2]。世界卫生组织发布的2019年全球十大死亡原因将其排名第三,因此加强COPD的防治势在必行。近年来,森林康养在促进大众健康和预防疾病方面受到社会各界的共同关注[3,4]。研究表明,森林有助于人们放松身心、调节机能、维持身心健康的作用[5]。我国学者也在森林康养方面也做了一些尝试[6,7],主要集中在单一的森林康养活动对疗养效果的影响[8],如森林浴能够降低军官血脂水平和改善老年人的高血压症状[9,10],能有效改善慢性阻塞性肺疾病(COPD)患者肺功能,同时增加患者运动耐力,提高体力状况[11,12] ,而短期的森林康养对COPD患者这一群体的影响实证研究鲜少见报。
依据慢性阻塞性肺疾病全球倡议 (GOLD),肺通气功能的指标主要包括慢肺活量(SVC)、深吸气量(IC)、第一秒用力呼气量(FEV1)、 用力肺活量(FVC)、一秒率、(FEV1/ FVC)、用力呼气25%瞬间流量(FEF25%)、用力呼气50%瞬间流量(FEF50%)、用力呼气75%瞬间流量(FEF75%)、最大呼气中期流量(FEF25-75%)、每分钟肺部通气量(MVV)。SVC和IC反映呼吸功能的潜在能力,与肺的弹性、胸廓功能及呼吸肌力量密切相关, FEV1和 FVC是测量人体肺通气功能的主要指标,FEV1反映气道阻力的变化,当气道阻力下降时,FVC提高;FEF25%反映呼吸早期的流量指标,与大气道功能密切相关,FEF75%反映呼吸后期的流量指标,常与FEF25-75%、FEF50%共同判断小气道阻塞或病变导致的功能障碍[13],当小气道阻力下降,FEF75%增加。在临床上,小气道病变被认为是COPD早期表现,也是慢性气管炎、支气管哮喘等许多肺部疾病的特征性表现[14]。MVV是呼吸困难和缺氧患者的重要指标,用以衡量胸廓肺组织弹性、气道阻力、呼吸肌力量。因此,通过对比COPD患者在短期森林康养前后肺通气功能主要指标的变化,分析短期森林康养活动对COPD患者肺通气功能的影响,旨在为评估森林康养效果提供相关方法和依据,丰富森林康养实证数据。

COPD患者在森林康养后肺功能各项指标均得到不同程度提升,其中FEF25-75%提升最多,FEF25%和FEF75%也有不同程度的提升,表明患者肺部小气道阻力下降,阻塞程度变轻;FEV1、 FVC和MVV的增加,表明患者肺部通气量增加,呼吸困难症有一定程度的改善(见表1)。因此,森林康养对COPD患者肺通气功能具有正向影响。

随着女性COPD病率的增加,探讨COPD的性别差异具有重要意义。有研究表明,COPD对女性的影响远比男性严重。如表2所示,森林康养活动对不同性别COPD患者肺功能指标的影响存在差异,短期的森林康养后,女性患者各项肺功能指标的提升率大于男性,表明森林康养对于女性COPD患者的作用大于男性COPD患者。

不同年龄阶段的COPD患者在短期森林康养后肺通气功能都得到了提升,提升率最多的为50~59岁年龄段,最小的为≥70岁年龄段(见表3)。但在60~69岁年龄段以及≥70年龄段,其提升率有显著的个体差异,与个体的身体状况以及肺通气功能受损程度密切相关。这可能是由于不同年龄阶段以及个体的呼吸系统气道和肺通气功能不同,对森林中的负离子以及植物精油吸收代谢功能不同,由此对其呼吸系统肺和气道的修复作用不同所致。

肺功能轻度受损者1人,森林康养后肺功能正常;中度受损者8人,重度受损者8人,极重度受损者3人,森林康养后肺功能均得到不同程度的提升,其中重度受损者肺功能各项指标提升率最高,其次为中度受损者,轻度受损者肺功能指标提升率最低(见表4)。

相关分析得出(见表5),用力呼气的瞬间流量FEF25% 、FEF50% 、FEF75%、 FEF25%-75%提升率与年龄呈显著(P < 0.05)或极显著(P < 0.01)的负相关关系,表明随着年龄的增加,森林康养对于COPD患者小气道阻力下降的作用减弱,阻塞程度变轻的程度减弱。MVV与肺功能受损程度显著负相关(P < 0.05),表明肺功能受损越严重的患者,经过短期的森林康养后,每分钟肺部通气量提升率越小。一秒率与肺功能受损程度显著正相关,表明患者肺功能受损程度越严重,森林康养改善一秒率的作用越大。

Short-term Forest Rehabilitation Influence on Pulmonary Ventilation in Patients with Chronic Obstructive Pulmonary Disease

Abstract: In order to study the effect of short-term forest rehabilitation activities on the pulmonary ventilation function of people with chronic obstructive pulmonary disease (COPD), 20 COPD patients living in urban areas during the remission period were selected to carry out a 5-day forest rehabilitation activity in Kuanba Forest Health Center. By comparing the changes of various indicators of pulmonary ventilation function before and after the patients' rehabilitation activities, the impact of short-term forest rehabilitation on the pulmonary ventilation function of COPD patients and the related factors were analyzed and discussed. The results showed that short-term forest rehabilitation had a significant positive effect on the recovery of pulmonary ventilation function indexes in COPD patients, and the effect on female patients was higher than that of male patients; the correlation analysis showed that the lifting rate of FEF25%, FEF50%, FEF75%, FEF25%-75% of the instantaneous flow of forced expiratory was significantly (P<0.05) or extremely significantly (P<0.01) negatively correlated with age, the MVV lifting rate was significantly negatively correlated with the degree of lung function damage (P<0.05), and the one-second rate was significantly positively correlated with the degree of lung function damage (P<0.05), indicating that short-term forest health care played a greater role in COPD patients with younger age and lighter lung function damage.
